 Applications on prescribed forms are invited from eligible Pakistani National candidates for

Senior House Officer (SHO) at Shaikh Zayed Federal Postgraduate Medical Institute, Lahore

Liver Transplant Surgery, Int. Medicine, Nephrology, Pulmonology, Psychiatry, Radiology,

Histopathology, Anatomy, Chemical Pathology, Dentistry, Accident & Emergency,

Pharmacology & Forensic Medicine.

Attach Attested Photocopies of the Following Documents: -

➤ MBBS degree & detail marks sheets of all professional examinations.

PM&DC/PMC valid permanent registration Certificate.

Copy of publications (if any) in approved journal from PM&DC/PMC/PubMed Indexed & HEC.

Domicile Certificate.

Computerized National Identity Card (CNIC).

Relevant subject/specialty related experience certificate from Public & Private Teaching

Hospital recognized from PMC for Postgraduation Training.

One passport size photograph duly attested from back side.

Those having foreign qualifications are required to submit HEC/PMDC/PMC equivalency

certificate.

Application and academic info form can be downloaded from the website,

www.skzmdc.edu.pk processing & application fee (Rs. 2,000) shall be submitted through online

Inter Branch Transection (IBT) in favour of Deputy Dean Shaikh Zayed Postgraduate Medical

Institute, Lahore Account No. 3154820902 National Bank of Pakistan (NBP) Shaikh Zayed

Hospital, Lahore Branch code 1707 and forms can be sent through couriers on the above cited

form address. Copy of Inter Branch Transection must be attached with application form.

Forms are also available from account section of FPGMI, Lahore on Payment of Rs. 2000/-.

Applicant cannot apply more than 02 departments.

The information given in the application form shall be verified at three (03) stages: -

During the submission & processing of application.

At the time of Acceptance/joining.

During the working

If information submitted is found wrong, mismatched or forged, the application will be rejected

and contract will be discontinued with no right of appeal at institution/government level.

> Applications received after due date or with incomplete information (Academic Info form) shall

not be entertained.
